REGISTRATION OF BIRTH OR DEATH OF FILIPINO CITIZEN IN THAILAND, OR MARRIAGE CONTRACTED BY A FILIPINO IN THAILAND
The birth or death of a Filipino citizen in Thailand, or the marriage contracted by a Filipino in Thailand must be reported at the earliest opportunity to the Philippine Embassy, Bangkok for proper registration with the Office of the Civil Registrar General in the Philippines.
Requirements for Report of Birth
- Personal appearance of at least one parent (the mother if the child is illegitimate);
- Report of Birth form duly accomplished;
- One (1) color ID picture of the child;
- Birth Certificate of the child translated into English and duly authenticated by the Thai Ministry of Foreign Affairs;
- Valid passports of the child’s parents and four (4) photocopies of each passport;
- Copy and four (4) photocopies of the Marriage Contract of the parents duly authenticated:
- If the marriage was solemnized in the Philippines, the marriage contract must be duly authenticated by the National Statistics Office (Office of the Civil Registrar General) and by the Department of Foreign Affairs (Authentication Division).
- If the marriage was solemnized under Thai law, the marriage contract must be accompanied by an English translation duly authenticated by the Thai Ministry of Foreign Affairs.
7. Payment of processing fee (THB1,125.00).
Requirements for Report of Marriage
- Marriage Contract translated into English and duly authenticated by the Thai Ministry of Foreign Affairs;
- Original Passport and photocopy of the passport’s personal data and visa pages;
- Four (4) copies of Report of Marriage form duly accomplished;
- Payment of processing fee (THB1,125.00).
Requirements for Report of Death
- Death Certificate issued by the appropriate Thai government agency translated into English and duly authenticated by the Thai Ministry of Foreign Affairs;
- Four (4) photocopies of the Death Certificate;
- Valid passport of the deceased;
- Report of Death form duly accomplished (4 sets); and
- Payment of processing fee (THB1,125.00).
Requirements and Precedures for Renewal of Philippine Driver's License
- Certificate of Employment
- Photocopies of Passport (first page, visa page, and pages showing departure in the Philippines and arrival overseas)
- Letter of authority to authorized representative; and
- Photocopy of driver's license and driver's license receipt (DLR), if availabe
