Amendments to Passport (applicable to green passport holders only)
Amendments are authorized changes made to a Filipino’s passport details as a result of a change in civil or legal status. An example is when a Filipino woman, after contracting marriage, decides to use the surname of her husband. Passports can be amended when a Filipino woman decides to revert to her maiden name after the death of her husband, after her marriage was annulled, or after being divorced by her foreign spouse.
The passport can also be amended to reflect a change of surname of a child who is legitimated by virtue of a subsequent marriage of his parents or, in case of adoption, to reflect the surname of the child’s adoptive father.
The specific requirements are as follows:
Amendment Due to Marriage
- Application form duly accomplished;
- Valid passport;
- If marriage was solemnized in the Philippines, original copy and photocopy of the marriage contract duly authenticated or certified true copy by the National Statistics Office (NSO) and the Department of Foreign Affairs;
- If marriage was contracted in Thailand: Original copy and photocopy of marriage contract issued by the appropriate Thai government agency translated into English and duly authenticated by the Thai Ministry of Foreign Affairs; Original Copy of the Legal Capacity To Marry issued by the Philippine Embassy; and, Report of Marriage form duly accomplished;
Amendment Due to Death of Husband
- Application form duly accomplished;
- Copy of the Death Certificate of the husband duly authenticated by the NSO and the Department of Foreign Affairs; [If the Death Certificate was issued by a Thai government agency, the Death Certificate must have an English translation and must be duly authenticated by the Thai Ministry of Foreign Affairs;
- Copy of the Birth Certificate of the husband duly authenticated by the NSO and the Department of Foreign Affairs;
- Valid passport; and,
Amendment Due to Annulment of Marriage
- Application form duly accomplished;
- Certified true copy of the Court Order effecting the annulment;
- Annotated Marriage Contract duly authenticated by the NSO and the Department of Foreign Affairs;
- Valid passport; and,
Amendment Due to Divorce
- Application form duly accomplished;
- If the divorce was obtained under Thai law, original copy and photocopy of the divorce decree issued by a Thai District Office translated into English and duly authenticated by the Thai Ministry of Foreign Affairs;
- If the divorce was obtained in other countries, a certified True Copy of the Divorce Decree duly authenticated by the Philippine Embassy or Consular Post which has jurisdiction over the place where the divorce was obtained;
- In case of a woman divorced by her Filipino Muslim husband, certified true copy of the Divorce Decree or a certified true copy of the Certificate of Divorce from the Shariah Court or the NSO;
- Valid passport; and,
Amendment Due To Adoption
- Application form duly accomplished;
- Valid passport;
- Copy of the Amended Birth Certificate duly authenticated by the NSO and the Department of Foreign Affairs;
- Copy of the Adoption Order of the Court:
Note: If adoption was obtained under Thai law, copy of the court order of adoption translated into English and duly authenticated by the Thai Ministry of Foreign Affairs
If adoption was obtained under Philippine law, copy of the court order duly authenticated or certified true copy by the issuing court, Department of Justice, Office of the President, and the Department of Foreign Affairs.
Amendment Due To Legitimation
- Application form duly accomplished;
- Valid passport;
- Copy of the marriage contract of the parents duly authenticated by the NSO and the Department of Foreign Affairs; and,
Note: If the marriage was contracted under Thai law, copy of the marriage contract translated into English and duly authenticated by the Thai Ministry of Foreign Affairs.
